Grades TK - 6th

2025-2026 TK Enrollment - Child will turn 4 on or before, September 1, 2025

2025-2026 Kindergarten Enrollment - Children will turn 5 on or before, September 1, 2025

ALL Enrollment Packets will be date/time stamped according to submission and MUST be submitted at the District Office - 305 Mark West Springs Rd.  Santa Rosa, CA 95404

Submission of an Enrollment Packet does not guarantee placement in the MWUSD

In addition to the printed and signed Online Enrollment Packet, a complete packet will include the following:

  • Proof of residency with your name on it (PG&E or Water Bill ONLY)
  • Copy of Child's Birth Certificate
  • Copy of child's immunization records
  • Completed Home Language Survey
  • Additional documents as noted on the "Documents" page during the Online Enrollment process 
  • If you live out of district, please also submit an Inter-District Transfer .  Please complete Section 1 of the agreement prior to submitting.
  • NOTE:  Students entering grades 1st -6th must also submit a copy of their 24/25 Attendance and Discipline Records from their current school.  If your student has no discipline record, a note, written on the school's letterhead and signed by a school official will suffice.

 

ALL completed Online Enrollment Packets must be submitted at the District Office - 305 Mark West Springs Rd., Santa Rosa between the hours of 7:30am - 5:30pm (Monday-Friday)

Please note: Beginning in the 2024-2025 school year,  ALL NEW students requesting an Inter-District Transfer to the Mark West Union School District entering grades 1st - 6th are REQUIRED to submit a copy of their Attendance and Discipline records from their previous school district with their required paperwork.  Consideration of enrollment will not be reviewed until all required documentation has been received.

2024-2025 Enrollment for NEW students entering grades TK-6th is OPEN!

Kindergarten: Child must turn 5 on or before September 1, 2024

TK: Child must turn 5 between September 2, 2024 - June 2, 2025

2024-2025 Enrollments must be submitted, in person, to the Mark West Union School District Office at 

305 Mark West Springs Rd., Santa Rosa.  

Note: An Enrollment packet is not considered complete until all documentation has been received

WHAT DOCUMENTS ARE REQUIRED FOR TK-6TH GRADE NEW ENROLLMENTS?

The online enrollment process is NOT CONSIDERED COMPLETE until ALL required documents have been received

  • Print and submit ALL documents listed as “required” in the “document” section of Online Enrollment

  • Print, sign and submit the Online Enrollment Application

  • A copy of your child’s Birth Certificate

  • Print A copy of your child’s Immunization Record

  • Proof of Residency at your current address (Only a copy of a PG&E or Water bill with parent(s) name(s) will be accepted)

  • ALL NEW students to the Mark West Union School District entering grades 1st-6th are required to submit a copy of their Attendance and Discipline Records from their previous school district.

All documents must be submitted to the Mark West Union School District Office at 305 Mark West Springs Rd., Santa Rosa.  Completed application packets should be delivered in person.

What should I do if I don’t have access to a computer/printer?  Will hard copies be available?

MARK WEST CHARTER SCHOOL

7TH GRADE PRE-ENROLLMENT FOR THE 2025-2026 SCHOOL YEAR

Limited spots are available and ALL PRE- ENROLLMENT PACKETS MUST BE ACCOMPANIED BY ALL REQUIRED DOCUMENTS IN ORDER TO RESERVE A SPOT FOR YOUR 7TH GRADE STUDENT IN THE 2025-2026 SCHOOL YEAR (WHILE SPACE IS AVAILABLE).  INCOMPLETE PACKETS WILL NOT RESERVE YOUR STUDENT'S SPOT.

ALL Pre-Enrollment Packets and supporting documents MUST be submitted directly to the Mark West Charter School Office at 4600 Lavell Rd. Santa Rosa, CA 95403

What do I need to bring to Pre-Enroll my 7th Grade student for the 2025-2026 school year?

  • Printed copy of the Online Enrollment Packet (signed/dated on the last page)
  • Jupiter Grades Form (available during Online Enrollment process - print/complete/turn in)
  • MWCS Admissions Info (print to ensure you have all requirements)
  • MWCS Parent & Student Pledge (available during Online Enrollment process - print/complete/return)
  • Parent/Guardian Application Questionnaire (available during Online Enrollment process - print/complete/return)
  • Student Application Questionnaire (available during Online Enrollment Process - print/complete/return)
  • Student Profile Completed by Current Teacher (Though not required at pre-enrollment, please print this document during the Online Enrollment process and submit to your student's current teacher. Once completed, the teacher may email the form to School Office Manager, Nikki Dale at ndale@mwusd.org )
  • Copy of Student's Birth Certificate
  • Copy of Student's Immunization Records (Note: Though not required at the time of pre-enrollment, verification of the TDap Booster is required by State law PRIOR to the student starting 7th grade.  If this verification has not bee submitted to the MWCS office prior to the start of 7th grade, student WILL NOT be able to attend school until the verification has been received)
  • Copy of Student's Discipline and Attendance reports from current school
